Official Rules
- NO PURCHASE NECESSARY. A PURCHASE WILL NOT INCREASE YOUR CHANCES OF WINNING. VOID WHERE PROHIBITED BY LAW. ALL FEDERAL, STATE AND LOCAL LAWS AND REGULATIONS APPLY.
- PARTICIPATION CONSTITUTES ACCEPTANCE OF RULES AND DECISIONS, CONSENT TO USE AND RELEASE OF CLAIMS. By entering this sweepstakes, each participant warrants that he or she has read and agrees to abide by all sweepstakes rules and that he or she is not prohibited from participating in the sweepstakes. Participation in this sweepstakes constitutes participant’s full and unconditional acceptance of these Official Rules and the decisions of Taymark, Inc.d/b/a Uniquely Quince (“Uniquely Quince”), its parents, affiliates, employees and agents, which are final, binding and non-negotiable in all respect. Each participant agrees to release, discharge, indemnify and hold harmless Uniquely Quince and its parents, affiliates, shareholders, employees, officers, directors, agents, representatives, advertising and promotion agencies and prize suppliers from any liability arising from or related to the sweepstakes, including without limitation, (a) unauthorized human intervention in the sweepstakes; (b) technical errors related to computers, servers, providers, or telephone or network lines; (c) printing errors; (d) lost, late, postage-due, misdirected, or undeliverable mail; (d) personal injury, death, and property damage; and (e) claims based on publicity rights, defamation, or invasion of privacy. Further, participants fully and unconditionally release all claims for consideration, copyright infringement and all other claims of any nature relating to the use of their entry, name or likeness.
- ELIGIBILITY. This sweepstakes is open to residents of the continental United States and Puerto Rico. Proof of age and identity must be furnished upon request. Employees, agents, independent dealers and affiliates of Uniquely Quince and its agents, affiliates and immediate family members of all such persons, are not eligible to participate in this sweepstakes. Uniquely Quince reserves the right, at its sole discretion, to cancel, terminate, suspend or amend this sweepstakes at any time for any reason. Uniquely Quince further reserves the right to amend these sweepstakes rules at any time without prior notice, at their sole discretion. Uniquely Quince reserves the right to disqualify any participant for any reason, including, but not limited to, violation of these Official Rules or infringement of the rights of a third-party.
- TO ENTER. This sweepstakes will run between September 1, 2007 and September 30, 2008. To enter this sweepstakes, participants are asked to go to http://www.uniquelyquince.com and complete the entry form by providing, first name, last name, phone number, email address, age, date of quinceanera, physical address, and how you heard about Uniquely Quince (“Eligible Entries”) to be eligible to win this sweepstakes. Please only one entry per person and e-mail address. By submitting an entry, participants agree to abide by these Official Rules. Persons submitting entries under false pretenses will be disqualified. Uniquely Quince is not responsible for telecommunications, network, electronic, technical or computer failures of any kind. Uniquely Quince is not responsible for any lost or misdirected mail sent by participant or Uniquely Quince.
- PRIZES. There will be one (1) potential winner randomly drawn from all Eligible Entries on or about the last day of each month. Chances of winning will depend on the number of entries received. Each winner will receive one tiara with velvet tiara box and silver name plate etched with “Mis Quince Anos”. The potential winner must sign a copy of these Official Rules, a form releasing Uniquely Quince from all claims, and a W-9 form, within 60 days in order to receive the prize. If a potential winner is a minor, his/her parent or legal guardian will be required to sign the required documents on the potential winner’s behalf. A winner is responsible for taxes any and all other costs and expenses not listed above. All decisions of the judges of this sweepstakes are final, binding and non-negotiable. No prize may be transferred or substituted
- IDENTIFICATION OF POTENTIAL WINNER. Uniquely Quince will notify the potential winners by e-mail or phone at the information provided on the entry within ten (10) business days following the drawing. The winner must accept the prize by signing and returning the Authorization and Release, W-9 Form and a copy of these sweepstakes rules no later than sixty (60) days from date of notification If a potential winner is a minor, his/her parent or legal guardian will be required to sign the required documents on the potential winner’s behalf.
The winner’s name will also be posted at http://www.uniquelyquince.com on or about the first Monday of each month. - SPONSOR: This sweepstakes is sponsored by Uniquely Quince having an address of Uniquely Quince, 4875 White Bear Parkway, White Bear Lake, MN 55110.
- MISCELLANEOUS. By entering this sweepstakes, participants consent to Uniquely Quince’s use of their name, likeness, image and biographical data for advertising and promotional purposes without additional compensation, except where prohibited by law. BECAUSE PERSONAL INFORMATION FROM PARTICIPANTS WILL BE COLLECTED BY ENTERING THIS SWEEPSTAKES, PARTICIPANTS SHOULD REVIEW Uniquely Quince’s Privacy Policy, which can be found at http://www.uniquelyquince.com/PrivacySecurity.cfm. By entering this sweepstakes, participant authorizes Uniquely Quince to contact participant for commercial purposes via fax, telephone, or e-mail addresses listed in the participant’s entry form. There will be no use of participants name or other personal information for future commercial purposes except as in accordance with Uniquely Quince’s privacy policy. This Agreement is governed by the laws of the State of Minnesota, without giving effect to conflicts of law rules.
AUTHORIZATION AND RELEASE

I represent that (a) I unconditionally agree to and accept the Official Rules of the Tiara of the Month Sweepstakes, (b) I have complied with the Official Rules, and (c) I have perpetrated no fraud or deception in entering the Sweepstakes or in claiming any prize.
I agree to release and hold harmless Taymark, Inc. d/b/a Uniquely Quince (“Uniquely Quince”), its related companies, and their respective officers, directors, employees, and agents (the “Released Parties”) from and against any claim or cause of action arising out my of participation in the Sweepstakes or receipt or use of any prize, including, but not limited to any injury, death, or damage to persons or property which may be caused, directly or indirectly, in whole or in part, from my participation in the Sweepstakes or receipt of any prize.
I hereby authorize Uniquely Quince to use my name for such purposes as it chooses, including, but not limited to, company publications, whether circulated to employees of the company or to others, training presentations, advertising, promotions and other trade or business uses. This authorization extends to all media including but not limited to print, video, and electronic media.
I release Uniquely Quince from all claims for consideration and all other claims of any nature related to the use of my name or likeness for such purposes.
